Ledging or ledge formation is one of the most common errors in endodontic instrumentation. Its the artificially created irregularity on the surface of the root canal wall that prevents the placement of the instruments to the apex of an otherwise patent canal.
Causes of ledging is the insertion of uncurved instruments short of the working length with excessive amount of apical pressure, I think skiping files during instrumentation is also a major cause. luckly NiTi rotary files have minimized this problem Tremendously.
