Prevention :
1) Good radiographic techniques should be used to determine accurately the apical constriction of the root canal.
2) Sound reference points should be used.
3) Stable instrument stops are perpendicularly placed to the shaft of the instrument.
4) All instruments are retained within the confines of the canal system.
5) The occlusion is reduced or refined befor working length determination and cleaning and shaping.
6) The working length is periodically verified when necessary.
7) Attention to detail must be paid during all cleaning and shaping procedures.
8) The integrity of the apical stop is assessed with stiff paper points or files.
